Free will affects Amir with this quote because when he chooses to run away, he is choosing to run away from Hassan. From a friend who had always been there for him and if had to would give up his life for him. From an opportunity to stand up for Hassan for once. From a chance not to be a coward and have the courage to stand up to Assef. Instead, Amir chose to turn his back on Hassan. As a result, Amir had so much guilt filling up inside him like a pain that just would not go away. He had felt horrible about himself, for knowing what happened to Hassan, and knowing he chose to do nothing about it. This decision to simply just run away had shaped Amir's life and unleashed a world of heartbreak, lies, and a guilty conscience for him.
